The Agreements and Amendments are published in PDF format and include form fields you may fill out first before you print them, rather than having to hand-fill each blank in the Agreement. Participant Agreement Documents in English

• SCRAM GPS Program Participant Agreement with Health & Safety Notice (English) – Use this for initial device assignment.
• SCRAM GPS Program Participant Agreement Amendment – Equipment Replacement (English) – Use this for device swap-outs.
• SCRAM GPS Program Participant Agreement Amendment – MC Consent to Contact (English) – Use this in addition to the SCRAM GPS Program Participant Agreement if your organization uses SCRAM GPS Monitoring Center services.
• Health and Safety Notice for SCRAM Systems Products – If you choose not to use the SCRAM GPS Program Participant Agreement with Health & Safety Notice provided above, then use this standalone document in conjunction with your in-house or custom-developed participant agreement.

• SCRAM GPS Program Participant Agreement Amendment – MC Consent to Contact (Spanish) – Utilice esto además del Acuerdo de participante del programa SCRAM GPS si su organización utiliza los servicios del SCRAM Systems GPS Monitoring Center.
• Aviso de salud y seguridad para productos de SCRAM Systems – Si decide no utilizar el Acuerdo de participante del Programa SCRAM GPS con el Aviso de salud y seguridad proporcionado anteriormente, utilice este documento separado junto con su acuerdo de participante interno o desarrollado a medida.
